Patient Policies

Payment & Insurance
- Payment is due at the time of service. We accept debit or credit cards only (no cash or checks).
- We will bill your insurance directly, but you are responsible for copays, coinsurance, and deductibles at the time of your visit.
- If Uniq Mind is not in-network with your insurance, you are responsible for charges not covered.
- A copy of your insurance card, whether original or photocopy, is considered valid.
Appointment Cancellations & No-Shows
- If you need to cancel or reschedule, please give us at least 48 business hours' notice to avoid fees.
- Missed appointments without notice (No-Shows) will result in a $115 fee.
- Cancellations made with less than 48 business hours' notice will result in a $75 fee.
- Two No-Shows or three Late Cancellations in a 12-month period may result in discharge from care.
- Appointment reminders are sent by phone, email, and text, but you are still responsible for keeping your appointment even if you do not receive a reminder.
Prescription Refills
- Patients must attend follow-up visits at least every three months to continue receiving prescription refills.
- Refills are not processed on an emergency basis and are handled only Monday through Thursday.
- Please request refills at least one week before you run out of medication and check with your pharmacy before contacting us.
Telemedicine Guidelines
- Telehealth visits are treated the same as in-person appointments. You must be in a private, quiet space, not in a vehicle or public area.
- Medications must be accessible during the session, and if the patient is a minor, a parent or guardian must be present.
- Failure to follow these guidelines may result in termination of the session, a No-Show mark, and associated fees.
Patient Portal
- Our Patient Portal makes it easy to complete forms, request refills, view billing, and message your provider securely.
- New patients will receive an email invitation to register before their first visit.
Code of Conduct
- We expect respectful participation in your care.
- Please cancel or reschedule at least 24 hours before your appointment if you cannot attend.
- If you are experiencing a crisis or having thoughts of harming yourself or others, call 911 or go to the nearest emergency room immediately. Uniq Mind is not a crisis center.
- Allow up to three business days for processing prescription refills or insurance updates.
- Concerns should be directed to our office manager respectfully.
- Disruptive behavior, including yelling or profanity, may result in discharge from care.
Out-of-State Patients
- Services are available only to Florida residents. A valid Florida ID and a Florida-based pharmacy are required.
- If you reside in multiple states, we will require proof of Florida residency, such as an ID or utility bill.
- Services may continue temporarily if you are visiting Florida, but ongoing care requires permanent residency.
- If you move out of Florida, you will be referred to a provider licensed in your new state unless your provider is licensed there as well.
Patient Rights
At Uniq Mind, you have the right to:
- Control access to your personal health information.
- Choose how we communicate with you.
- File a complaint if you believe your information was disclosed inappropriately.
- Refuse treatment or request a clear explanation of risks and alternatives.
- Receive care in a safe, respectful environment.
- Be free from drugs, alcohol, or illicit substances during visits.
