Patients or family members wishing to refer should use our patient referral page or contact the office directly.
Most patients are scheduled within seven to ten business days of referral. Patients eligible for secure telehealth are often seen sooner. Urgent referrals are triaged based on clinical need — please flag urgency in the form.
Patient demographics and contact info, working diagnosis, current medications and prior medication trials, presenting concern, urgency, preferred service type (medication management, therapy, evaluation, advanced treatments), and your contact info for updates. The more clinical context you provide, the better we can match the patient with the right clinician.
Yes. With patient consent we confirm receipt of your referral, send a note after the initial evaluation, and provide periodic progress updates. You can request fax or secure email updates when submitting the referral.
