The reasons they waited are remarkably consistent: they didn't feel "bad enough," they thought they should be able to handle it on their own, they were worried about being judged, or they didn't know what an evaluation would actually involve.

The threshold for scheduling a mental health evaluation is much lower than people think. You don't need to be in crisis. You don't need to have a clear diagnosis in mind. You don't need to know whether you want medication. The point of the evaluation is to figure out what's going on — not to confirm a problem you've already named.

Patterns Worth Taking Seriously

Symptoms that have lasted more than a few weeks. Persistent low mood, anxiety, irritability, sleep changes, appetite changes, or fatigue lasting two or more weeks is worth evaluating. Acute stress responses pass; clinical conditions tend to settle in.

Functioning is affected. Work performance is slipping. Relationships feel harder. Hobbies you used to enjoy don't pull you in. Things that should be simple — getting out the door, returning a phone call, finishing a task — feel disproportionately heavy. Functional impairment is one of the clearest signals.

You've started avoiding things. Social events, errands, situations that previously felt fine. Avoidance is one of the most reliable markers of anxiety conditions and a frequent feature of depression as well.

The visit covers your current symptoms and how long they've been present, your medical and psychiatric history, any prior treatments and how they went, your family history, your medications, substances, and the parts of your daily life — work, relationships, sleep, exercise — that affect how you're doing.

Standardized rating scales are typically used to anchor what we're hearing in measurable data. We complete the visit either with a working diagnostic picture and a plan, or with a clear next step toward gathering more information if more is needed.

The plan that comes out of the evaluation may include therapy, lifestyle adjustments, medication, a referral for additional assessment, or a combination. Medication management is one tool among several — not a foregone conclusion.

What Happens If You Need Medication

If medication is appropriate, we explain what we're recommending, why, what the expected timeline is, and what side effects to watch for. We start at conservative doses, schedule a follow-up to track response, and adjust based on what you're actually experiencing. The goal is the smallest effective dose of the medication that fits you best.

If standard medication trials haven't worked in the past — or you've had unusual reactions — pharmacogenomic testing may be part of the plan. If conventional medications aren't fitting after adequate trials, treatments like TMS or ketamine become part of the conversation.

Most initial psychiatric evaluations are scheduled for 60 to 90 minutes to allow for a thorough assessment. Follow-up visits are shorter and focused on treatment response and adjustment.

Does what I share stay confidential?

Yes. Clinical conversations are protected by HIPAA. Information is not shared with employers, family members, or outside parties without your written authorization, with narrow legal exceptions for imminent safety concerns.
